A glycoprotein enzyme present in various organs and in many cells. The enzyme catalyzes the hydrolysis of a 5'-ribonucleotide to a ribonucleoside and orthophosphate in the presence of water. 5' Nucleotidase testing provides supportive evidence in the diagnosis of liver disease. 5'-nucleotidase is elevated in liver disease, hepatic carcinoma, biliary cirrhosis, extra-hepatic obstruction and metaplastic metaplasia of the liver and usually is not elevated in skleletal disease. 5' Nucleotidase test is specific for cholestasis or damage to the intra or extrahepatic biliary system, and in some laboratories, is used as a substitute for GGT for ascertaining whether an elevated ALP is of biliary or extra-biliary origin.
