Total iron-binding capacity is a test that measures the extent to which iron-binding sites in the serum can be saturated. Because the iron-binding sites in the serum are almost entirely on circulating transferrin, this is really an indirect measurement of the amount of transferrin in the blood. Along with with serum iron and percent transferrin saturation this test is performed when anemia or iron deficiency are suspected. However, because the liver produces transferrin, liver function must also be considered when performing this test. It can also be an indirect test of liver function, but is rarely used for this purpose. The liver produces more transferrin in response to iron deficiency, so the percent transferrin saturation (i.e., the result of the formula of serum ironTIBC x 100) can also be a useful indicator. Total Iron Binding Capacity (TIBC) correlates with serum transferrin. TIBC is increased in iron-deficiency anemia, oral contraceptive use, and in late pregnancy. TIBC is decreased in hypoproteinemia, chronic infections, hemochromatosis, cirrhosis, neoplastic and renal disease, and thalassemia.
