Description
Urobilinogen is a colorless tetrapyrrol that is formed from the reduction of bilirubin by intestinal bacteria. Roughly 20% of the urobilinogen is reabsorbed, returns to the liver via the portal vein, and is excreted by the kidneys in the form of urobilin. The remainder is excreted in the stool. The three mechanisms that alter bilirubin metabolism and can increase or decrease urobilinogen concentration are the following: 1) pre-hepatic overproduction of heme due to diseases such as sickle cell anemia, hemolytic anemia, transfusion reactions, thalassemia, and pernicious anemia, which leads to an increased production of bilirubin and later, urobilinogen; 2) hepatocellular disorders in which the hepatocytes' ability to take up bilirubin, conjugated to glucuronic acid and excreted into the bile duct is impaired which causes a decreased urobilinogen concentration; and 3) post-hepatic obstruction of the bile duct and the biliary system so that conjugated bilirubin is unable to enter the intestines and no urobilinogen is produced.(Fundamental of Urine & Body Fluid Analysis, Third,Nancy A.
