Wuchereria bancrofti, Brugia malayi and Brugia timori are the three species of filaria that cause lymphatic filariasis in humans living in the tropics and sub-tropics. Infection is spread from person to person by mosquito bites. The adult worms live and reproduce in the human lymphatic system, creating millions of microscopic worms called microfilariae. When a person who is infected is bitten by a mosquito, the mosquito becomes infected, and the microfilariae mature inside the mosquito. When the mosquito bites another person, the larval worms are passed from the mosquito to the human and travel to the lymphatic system, where they grow into adult worms. Most people with lymphatic filariasis are asymptomatic, and a few develop lymphedema, primarily in the legs, but also in the arms, breasts, and genitals. People with lymphedema are more susceptible to bacterial infections of the skin and lymphatic system, which can lead to hardening and thickening of the skin, also known as elephantiasis.
Filarial infection can also cause tropical pulmonary eosinophilia syndrome. Lymphatic filariasis primarily develops in people who have gotten mosquito bites in an endemic area over an extended period of time. IgG1, IgG2 and IgG3 are most common in asymptomatic patients, while IgG4 is a marker of active infection.
