Description

Hypersegmented neutrophils are neutrophils with 5 or more nuclear lobes. They are often seen in disorders of nuclear maturation and are thought to be pathognomonic of the class of anemias called megaloblastic anemias. Some studies, however, have strongly associated neutrophil hypersegmentation with iron deficiency anemia.
One of the earliest changes in the megaloblastic processes is the appearance of these hypersegmented neutrophils. Because of the short life-span of neutrophils, these abnormal cells characteristically appear even before the onset of anemia [2]. However, such neutrophils are not seen in the other classes of anemias, which incidently are far more common than megaloblastic anemias. It is of note that pernicious anemia is a type of megaloblastic anemia, and as such, may show hypersegemented neutrophils.
