Cryoglobulins are immunoglobulins that precipitate as serum is cooled below core body temperatures. Cryoglobulinemia is characterized by the presence of cryoglobulins in the serum. Single-component cryoglobulins contain only one isotype of immunoglobulin or, in rare instances, a cryoprecipitable monoclonal immunoglobulin light chain. Mixed cryoglobulins are immune complexes made of two different types of immunoglobulin, usually an IgM antiglobulin and IgG antigen. The classification initially proposed (Brouet et al) describes simple cryoglobulins as type I, and sub-divides mixed cryoglobulins as type II, in which case the antiglobulin is typically monoclonal, and type III, in which the antiglobulin is polyclonal. Some disorders associated with Type I are: multiple myeloma, Waldenstrom's macroglobulinemia, and other lymphoproliferative diseases. Type II may be associated with: chronic hepatitis C virus infection, Sjogren's syndrome, Waldenstrom's macroglobulinemia, chronic lymphocytic leukemia, non-Hodgkins' lymphoma, autoimmune diseases. Type III may be associated with: chronic infections, viral (EBV, CMV, HIV, hepatitis viruses), bacterial (SBE, leprosy, spirochetal), fungal, parasitic, autoimmune diseases, systemic lupus erythematosus (SLE), rheumatoid arthritis, inflammatory bowel diseases, and biliary cirrhosis. Screening for complement levels is done by quantitating antigenic levels of C3 and C4 and determining total hemolytic complement (CH50). Very low or undetectable levels of C4 with normal or relatively normal levels of C3 are often seen in type II cryoglobulinemia, and may suggest this diagnosis or a need for further workup for HCV infection. Low complement levels may also be seen in association with type II cryoglobulinemia complicating Sjogren's syndrome or with type III cryoglobulinemia in patients with rheumatoid vasculitis or SLE.
