Description
The aspartate aminotransferase (AST) to platelet ratio index (APRI) was originally published as a marker of significant fibrosis and cirrhosis in patients with hepatitis C. More recently, it has also been studied in patients with chronic hepatitis B. The APRI is calculated from two common lab results as follows: [(AST/upper limit of normal range for AST)/platelet count]x100. For cirrhosis, a score of greater than 2 is considered positive while patients with a score of less than 1 are considered unlikely to have cirrhosis. For significant fibrosis, 1.5 is used as the threshold for identifying significant fibrosis, and less than 0.5 rules it out. Other indirect indicators of liver fibrosis and cirrhosis include the FIB-4, which is based on AST, ALT, platelet count, and patient age, and the Forns index, which uses GGT, cholesterol, platelet count, and patient age.
