Description

Transthyretin (TTR) (originally called prealbumin or thyroxine-binding prealbumin) is a homotetrameric unglycosylated tryptophan rich protein that carries the thyroid hormones T3 and T4, and retinol in serum and CSF. Synthesized in hepatocytes, TTR and has a molar mass of 55000 daltons. At a pH of 8.6, an electrophoretic band appears prior to albumin in a relative amount of < 2.5 % due to its greater rate of diffusion to the anode. Its function is to bind and transport low molecular weight retinol-binding proteins, preventing their glomerular filtration. 30 50 % of circulating TTR is complexed by retinol-binding protein. Additionally, it binds and transports thyroxine (T4), nevertheless its affinity to this hormone is less than that of thyroxine binding globulin. TTR is decreased in hepatocellular synthesis caused by acute liver damage, or dietary protein deficiency, and because of its short half life (approximately 2 days) is used as a surrogate marker of nutritional status. TTR is also a negative acute phase reactive protein and levels decrease in inflammatory states. Levels can be elevated in chronic renal failure, high concentrations of corticosteroids, and Hodgkin's disease.
