Urinary cortisol reflects the portion of serum-free cortisol filtered by the kidney, freely filtered. It correlates with cortisol secretion rate. Source: Regenstrief Institute

Urinary free cortisol is a measurement of the cortisol in the urine that is not attached to other substances. Free cortisol represents the active form of the hormone. The urine measurement directly reflects the blood level of cortisol. Source: Regenstrief Institute

Cortisol is excreted into the urine unbound, that is, as free cortisol. In most cases this is the only form excreted and measured in the urine. Urinary-free cortisol concentration closely approximates the unconjugated plasma cortisol concentration. Whereas serum cortisol levels fluctuate throughout the day and are dependent on concurrent cortisol-binding globulin (CBG) levels, a 24-hour urine measurement integrates the cortisol production for the entire day and is not affected by CBG. The greatest value of urinary free cortisol measurement is to detect states of adrenal hyperfunction such as Cushing's syndrome. Source: Regenstrief LOINC
