Description
Alpha-1-microglobulin (also called protein HC or A1M) is a 31-kD, single-chain plasma glycoprotein that may be involved in regulation of the inflammatory process. The alpha-1-microglobulin/bikunin precursor gene (AMBP) codes for a precursor that splits into alpha-1-microglobulin, which belongs to the lipocalin superfamily, and bikunin. A1M is normally present as a free form and as a complex with IgA in the blood, spinal fluid, and urine. A1M inhibits neutrophil chemotaxis to endotoxin-activated serum. [OMIM: 176870] Abnormal plasma/serum levels can be seen in liver or kidney disorders, and also in clinical conditions such as HIV and mood disorders. Urinary A1M is used for the early detection and monitoring of kidney tubular disorders such as heavy metal intoxications, diabetic nephropathy, urinary outflow disorders or pyelonephritis). PMID: 15256311 Increased urinary excretion of α1-microglobulin in tubular proteinuria indicates reduced tubular reabsorption under normal glomerular filtration conditions. This form of proteinuria is typical for chronic interstitial nephropathy and for acute and chronic tubular damage caused by endogenous and exogenous tubular toxins. In renal failure, plasma levels of this microprotein increase from an early stage. The resultant protein hyperfiltration in the residual nephron causes increased renal excretion as re-absorption capacity is exceeded. α1-Microglobulin can be used as a marker to diagnose tubulo-interstitial nephropathy, for example, at an early stage, or rule it out with a high degree of certainty. Acute and chronic forms of tubular insufficiency (all forms of primary and secondary Fanconi syndrome), heavy metal intoxication, nephrotoxic side-effects of pharmaceuticals, and rejection reactions following kidney transplantation can also be excluded.
