Description

Distinguishing between large B-cell lymphomas is critical because the different forms often require distinct management. Diffuse large B-cell lymphoma (DLBCL) is the most common form, making up 30-40% of all non-Hodgkin's lymphomas. DLBCL is defined pathologically by large neoplastic cells with B-cell derivation and clinically by aggressive presentation, but differs from its former putative subtype, primary mediastinal large B-cell lymphoma (PMBCL), which is believed to arise from thymic medullary B cells. Since 2001, the WHO has recognized PMBCL as a distinct clinicopathologic disease from DLBCL, with likely distinct treatment options. PMBCL accounts for approximately 2% of patients with NHL, and tends to affect younger patients. PMID: 16720849. PMBCL is generally not further subclassified during a patient workup, whereas DLBCL can be further subdivided by cell of origin (COO) into germinal center type and non-germinal center (activated B-cell) type, the latter having significantly worse response to certain treatments PMID: 25135992.
