A procollagen peptide contains an additional peptide sequence, termed the propeptide, at its amino and carboxy terminal ends. Collagen types I, II, III, IV and V are synthesized as precursor molecules called procollagens. Propeptides facilitate the winding of procollagen molecules into a triple-helix conformation within the endoplasmic reticulum. These propeptides are then cleaved from the collagen triple helix molecule during its secretion, after which the collagens polymerize into extracellular collagen fibrils. Thus, the amount of free propeptides directly correlates with collagen molecule synthesis. Procollagen Type I C-peptide (PIP) has also been extensively referenced in correlation studies between collagen levels and certain health disorders such as bone disease, alcoholic liver disease, liver cirrhosis and cirrhosis (Borrmann type IV) adenocarcinoma of the stomach. Procollagen type I N-terminal propeptide is considered a sensitive marker of bone formation and it is particularly useful for monitoring treatment for osteoporosis in postmenopausal women, and Paget's disease of the bone. Type I collagen constitutes 90% of bone proteins and is synthesized as type I procollagen. During the extracellular processing of type I procollagen, the amino terminal [N-terminal propeptide of type I collagen] and carboxy terminal propeptide are separated. These propeptides circulate in the bloodstream, where they are markers of bone formation. Procollagen type I N-terminal propeptide circulates as different forms, including the intact authentic trimeric procollagen type I N-terminal propeptide, a monomer, and several fragments.
