Reticulated platelets (RPs) are the youngest circulating platelets and have been shown to reflect thrombopoietic activity in humans. They are identified by their increased RNA content. An elevated percentage of RPs is diagnostic of increased marrow platelet production, such as in response to idiopathic thrombocytopenic purpura. A low percentage of RPs is associated with hypoproliferative causes of thrombocytopenia, including chemotherapy or intrinsic marrow aplasia. In addition to the circulation percentage of RPs by RNAse-based analysis, the RP maturation index (RP-MI), or immature platelet fraction, further refines the sensitivity of RP detection and may be useful to predict platelet count recovery following chemotherapy-induced thrombocytopenia. Platelets freshly released from the bone marrow contain more RNA (and cell counters can identify them as such with fluorescent RNA stains) Immature platelts are more hemostatically active, and are seen in higher percentages among diabetics and patients with acute coronary syndromes.
