Term Description

The Hendrich II Fall Risk Model ® is a predictive fall risk model used to screen and identify patients that are at an increased risk of falling as part of a holistic assessment. A nurse or care provider will evaluate the patient for the eight risk factors during each shift and/or whenever their condition changes to update the fall risk score. A fall risk score is calculated from the findings for each statistically weighted risk factor. The tool is validated for use in patients 18 years and older (except Obstetrical patients). It can be applied in all hospitalized patients, in addition to the emergency department, ambulatory areas, clinics, skilled nursing, home, assisted living, and rehabilitation settings.
