CA 125 is an antigenic determinant associated with a mucin-like glycoprotein, molecular weight exceeding 500,000 Daltons. This determinant is recognized by the murine monoclonal antibody OC 125, which reacts with over 80% of epithelial ovarian cancers. Elevated CA 125 levels have been reported in subjects with serous, endometrioid, clear cell, and undifferentiated ovarian carcinoma adenocarcinoma of the endometrium or fallopian tube, as well as some nongynocologic malignancies and certain nonmalignant conditions. Elevated levels also have been observed in pregnant women, most often during the first trimester. CA 125 levels may have significant value in monitoring ovarian carcinoma patients. A persistently rising CA 125 value may be associated with progressive malignant disease and poor therapeutic response. A declining CA 125 value is indicative of favorable prognosis and good response to treatment. CA 125 levels may help predict whether an invasive second-look procedure such as laparoscopy or laparotomy is required after treatment for epithelial ovarian cancer.
