Ki-67 is a proliferation marker that correlates with flow cytometric S-phase. Breast cancer specimens with greater than 20% Ki-67 positivity will generally have a high S-phase fraction, which is an unfavorable prognostic marker.

Ki-67 nuclear antigen is a marker of cell proliferation. In breast cancer specimens, the degree of Ki-67 staining, assessed using MIB-1, a monoclonal antibody, is associated with the degree of proliferation. In general, tumors with higher degrees of proliferation are associated with worse prognosis. While the baseline degree of Ki-67 staining may not be predictive, the change in the proportion of Ki-67 positive cells before and after therapy may be predictive of disease-free and overall survival.
