Aspartate aminotransferase (AST) is an enzyme that is released by injured muscle cells and liver cell and is predominately found in the liver and heart. AST levels in the blood are most often used to monitor liver damage. AST is typically tested in conjunction with alanine aminotransferase (ALT) when a liver panel is ordered. High levels of AST in the bloodstream could indicate hepatitis, drug damage to the liver, cirrhosis, alcoholism, or other liver disorders. [Lab Tests Online:ast/tab/test]

Pyridoxal phosphate is a derivative of Vitamin B6 and is used by enzymes in all organisms. The majority of enzymes which depend on pyridoxal phosphate are associated with pathways involving amino acids, such as amino group transfer, decarboxylation and other important biochemical processes. When measuring AST and/or ALT without pyridoxal-5-phosphate (P-5-P), the rate of the decrease of NADH is directly proportional to the AST/ALT (respectively)-activity in the specimen. Testing for AST and/or ALT with the addition of P-5-P stabilizes the transaminases in the specimen and avoids falsely low values in specimens of patients suffering from either myocardial infarction or liver disease, who have an insufficient amount of P-5-P in their blood.
