The activated clotting time (ACT) measures the time to clot formation after addition of an activator, and is used primarily to monitor a patient's coagulation status during surgery, especially procedures during which patients are on cardiopulmonary bypass. The activator may stimulate the intrinsic and/or extrinsic coagulation pathway, and commonly used activators include kaolin, Celite and glass beads. Various anticoagulant drugs can be used to prevent clot formation and consumption of platelets during surgery. The most common is unfractionated heparin because it has a short half-life and is easily reversed. Baseline ACT and subsequent ACT measurements during and after the procedure help determine the initial heparin dose and titration parameters to achieve a stable level of anticoagulation during surgery, as well as appropriate reversal of anticoagulation after surgery. The ACT can be affected by platelet count, platelet function, temperature of the blood, and acquired or inherited hematologic conditions. The ACT is a point of care test used to monitor patients receiving high doses of heparin in an acute setting, while partial thromboplastin time (PTT) is used to monitor patients who are on lower, long-term therapeutic doses of heparin.
