ProBNP is the 108 amino acid pro-hormone of BNP (Brain Naturetic Peptide) that is produced mainly in the left ventricle. The prohormone splits into two polypeptides- the biologically active but shorter BNP (77-108) and the longer N terminal (1-76) fragment called NT-proBNP. Commercial assays are available for NT-proBNP because of its usefulness in predicting cardiovascular risk. In one study, it was the single best predictor of survival among patients with the acute coronary syndrome. It also declines with successful treatment of left ventricular dysfunctionand heart failure and is used by some to track the success of such treatment. No commercial assays exist for proBNP (the whole peptide)- though the trade name for one companies NT-proBNP is "proBNP" -- a misnomer.
