Testosterone is an androgenic steroid and is the primary male sex hormone. Approximately half of circulating testosterone is tightly bound to a protein called sex hormone binding globulin (SHBG) and is biologically inactive. Another fraction is weakly bound to other proteins, such as albumin, and the remainder is unbound, or "free". Weakly bound and free testosterone together represent bioavailable testosterone. Both total and free testosterone levels decrease with age. Low testosterone is associated with many other conditions, including osteoporosis, Alzheimer's disease, obesity, diabetes and ischemic heart disease. A potent androgenic steroid and major product secreted by the Leydig cells of the testis. Its production is stimulated by luteinizing hormone from the pituitary gland. In turn, testosterone exerts feedback control of the pituitary LH and FSH secretion. Depending on the tissues, testosterone can be further converted to dihydrotestosterone or estradiol. A triple bolus test or a dynamic pituitary function test is a medical diagnostic procedure used to assess a patient's pituitary function. Three hormones (usually synthetic analogues) are injected as a bolus into the patient's vein to stimulate the anterior pituitary gland: insulin, gonadotropin-releasing hormone (GnRH), and thyrotropin-releasing hormone (TRH). The gland's response is assessed by measuring cortisol, growth hormone (GH), prolactin, thyroid-stimulating hormone (TSH), luteinizing hormone (LH) and follicle-stimulating hormone (FSH) hormone levels. Blood glucose levels are also monitored for hypoglycemia. A triple bolus test is usually ordered and interpreted by endocrinologists.
