Luteinizing hormone (LH) is a hormone synthesized and secreted by gonadotropes in the anterior lobe of the pituitary gland. In concert with the other pituitary gonadotropin follicle stimulating hormone (FSH) it is necessary for proper reproductive function. In females, an acute rise of LH triggers ovulation and in males, where LH had also been called Interstitial Cell Stimulating Hormone (ICSH), it stimulates Leydig cell production of testosterone.

Lutropin is a glycoprotein pituitary hormone, which is under complex regulation by hypothalamic gonadotropin releasing hormone (GnRH) and by gonadal sex hormones, estrogen and progesterone in females, and testosterone in males. High concentrations of LH (during the follicular phase) in patients with polycystic ovary syndrome interfere with conception, and may contribute to early pregnancy loss in these patients. In males, LH has been called interstitial cell stimulating hormone (ICSH) because of its effect on testosterone production by Leydig cells. This is necessary for the normal maturation of spermatozoa. LH measurement is used in evaluating the normalcy of hypothalamic-pituitary-gonadal axis. Measuring serum gonadotropin levels allows for distinguishing between primary gonadal failure and deficient gonadal stimulation. LH measurement may be of clinical importance because growth hormone and LH are frequently the first hormones to be affected by pituitary disease. Serum analysis of LH has also been found to be useful in the diagnosis and treatment of infertility in women. Secretion of LH and FSH (Follicle-Stimulating Hormone) is pulsatile in response to normal intermittent release of GnRH. While both are pulsatile, LH exhibits circadian rhythm while FSH does not. In females both LH and FSH vary over the course of the menstrual cycle, with peaks at ovulation.

A triple bolus test or a dynamic pituitary function test is a medical diagnostic procedure used to assess a patient's pituitary function. Three hormones (usually synthetic analogues) are injected as a bolus into the patient's vein to stimulate the anterior pituitary gland: insulin, gonadotropin-releasing hormone (GnRH), and thyrotropin-releasing hormone (TRH). The gland's response is assessed by measuring cortisol, growth hormone (GH), prolactin, thyroid-stimulating hormone (TSH), luteinizing hormone (LH) and follicle-stimulating hormone (FSH) hormone levels. Blood glucose levels are also monitored for hypoglycemia. A triple bolus test is usually ordered and interpreted by endocrinologists.
