LP36372-8 Bile acid fractions panel
Bile acids (BAs) play a primary role in digesting dietary fats. Primary BAs are produced in the liver by either the classic or alternative pathway. CYP7A1 controls the production of primary BAs in the liver. The primary BAs (chenodeoxycholic acid and cholic acid) are conjugated to either glycine or taurine to create conjugated primary BAs, which are stored in the gallbladder. Once released into the intestinal circulation, primary BAs are modified by gut bacteria (e.g., deconjugated) to form secondary BAs. Secondary BAs are actively reabsorbed in the terminal ileum, and travel back to the liver through the portal circulation. PMID: 28249284
Different disorders are associated with various patterns of primary and secondary BA levels. Examples of disorders and their associated BA patterns include:
Disorders of BA synthesis (information from PMID: 18577977):
1) Inborn errors of bile acid synthesis including cerebrotendinous xanthomatosis, Zellweger syndrome, and a variety of other enzyme deficiencies - low or absent BAs
2) Bile acid conjugation defects - increased levels of unconjugated cholic and deoxycholic acid, lack of chenodeoxycholic acid
Disorders of BA metabolism (information from PMID: 28249284):
1) Inflammatory bowel disease - reduced deconjugation by gut bacteria, lower levels of secondary BAs
2) Irritable bowel syndrome - reduced deconjugation, increased levels of primary conjugated BAs, lower levels of secondary BAs
3) Short bowel syndrome - reduced levels of secondary and tertiary BAs
4) Obesity - lower levels of both primary BAs
