HIV enters cells by attaching first to the CD4 receptor and then to either the CCR5 or CXCR4 molecules, followed by fusion of the viral and cellular membranes. CCR5 coreceptor antagonists prevent HIV entry into target cells by binding to the CCR5 receptors. HIV tropism assays determine whether the HIV binds to CCR5, CXCR4 or both. An HIV tropism assay should be performed whenever the use of a CCR5 antagonist is being considered or a patient has virologic failure on a CCR5 antagonist.
During acute or recent infection, the vast majority of patients harbor a CCR5-binding virus (called R5 tropism), which suggests that the R5 variant is preferentially transmitted. Viruses in many untreated patients exhibit a shift in co-receptor tropism from CCR5 usage to either CXCR4 (X4 tropism) or both (D/M-tropic, which stands for dual or mixed), which is associated with a more rapid decline in CD4 T-cell counts. Patients who have been treated with anti-retroviral drugs and who have extensive drug resistance are more likely to harbor X4- or D/M-tropic variants than untreated patients with comparable CD4 counts.
