Testosterone is an androgenic steroid and is the primary male sex hormone. Approximately half of circulating testosterone is tightly bound to a protein called sex hormone binding globulin (SHBG) and is biologically inactive. Another fraction is weakly bound to other proteins, such as albumin, and the remainder is unbound, or "free". Weakly bound and free testosterone together represent bioavailable testosterone. Both total and free testosterone levels decrease with age. Low testosterone is associated with many other conditions, including osteoporosis, Alzheimer's disease, obesity, diabetes and ischemic heart disease. Gonadotropin releasing hormone (GnRH) is a hormone secreted by the hypothalamus in order to stimulate the release of luteinizing hormone (LH), follicle stimulating hormone (FSH) and gonadotropins from the pituitary gland. GnRH release is crucial to the reproductive cycles of both males and females. In males, GnRH is instrumental in the production of sperm cells in the testes. In females, GnRH is necessary for conception and regulation of the menstrual cycle. To ensure the pituitary is coordinating properly with GnRH, an LH response to GnRH blood test is conducted. In this test, a shot of GnRH is administered to a patient and the LH level is observed over a specified period of time.
