Term Description
A public health case report document contains common or standardized data elements for reportable conditions, such as hepatitis B, tuberculosis, and tularemia. Healthcare facilities or providers may use this document to send reportable condition data to state and local public health departments. Data elements include the patient's immunization records, encounters, diagnostic testing, social history, and treatments. This term was originally created for, but not limited in use to, the HL7 Implementation Guide for CDA R2: Public Health Case Reports.
