Term Description

A document that summarizes all of the currently prescribed medications for a patient at a given point in time (such as the end of a hospitalization or other encounter) as known by the document's author. It may contain additional medication-related information such as allergies. The purpose of this document is to inform the patient and other clinicians (therapists, etc) of the current medication information.
