Cortisol is a glucocorticoid produced in the adrenal cortex. A synthetic version of this same chemical is called hydrocortisone and manufactured as medication. Cortisol is not to be confused with cortisone, a closely related compound with much less (PubChem calls it "inactive") biologic activity. Cortisone is also offered as a drug, and it is converted to cortisol (the active form) in the liver. They both have a circadian rhythm with peak in the AM (around 8AM) and trough in the evening (around midnight), but this shifts according to sleep habits (work shifts). The morning measurement, which should be high, is most useful for diagnosing Addison's disease (too little cortisol) and the evening measurement, which should be low, is the most useful for diagnosing Cushing's disease (too much cortisol). Salivary cortisol may be a more sensitive measure than serum or plasma.

LP7565-7 Saliva
Saliva is produced primarily by the parotid, submaxillary, and sublingual glands. It is secreted at the rate of 0-3 milliliters per minute which is affected by various factors including anxiety, hydration and hunger. Oral fluid is broadly used to test for drugs of abuse in employment and forensic settings, and is especially useful since it is difficult to adulterate, is quick and is non-invasive. Oral fluid is used to test a variety of analytes, including to test alcohol, HIV and other antibodies, therapeutic drugs and steroids, but its primary use is to test for drugs of abuse including amphetamines, cocaine and metabolites, opioids, methadone, cannabis and heroin.
