Albumin is a predictor of morbidity and mortality in dialysis patients. Bromocresol green (BCG) and bromocresol purple (BCP) dye binding methods differ enough in results for serum albumin in hemodialysis patients to necessitate distinct LOINC codes.

BCG reacts with alpha-1 and alpha-2 globulins, as well as albumin, whereas BCP appears to react more specifically with albumin. The consequence is that BCG results will be higher than BCP results. See attached paper which shows a consistent difference of approximately 5.5 g/L in all classes of patients - not just nephrology/dialysis patients. Clase et al, Neph Dial Transplant, 2001;16:1925-9. This is the only paper we found that that compared BCG and BCP in any significant number of non-renal patients. There are other arguments about these two methods in renal dialysis patients. Both hemodialysis and peritoneal dialysis patients commonly have an inhibitor (CMPF) that competes with the dye for albumin binding sites. Doumas and Peters, Clin Chem 2009;55:583-584 (this is a reprint of a "citation classic", originally published in Clin Chim Acta 1971;31:87), point out the differences between BCG and BCP albumin results, and state that is essential for laboratories to establish separate reference ranges for each, and to advise their clinical customers of the differences.

A non-glycosylated protein that is synthesized in the liver parenchymal cells, regulated by colloid osmotic pressure from interstitial fluid surrounding hepatocytes, and catabolized in nearly all organs. Albumin comprises >50% of plasma proteins in plasma, CSF and urine, and therefore is a key contributor to intravascular oncotic pressure and regulating fluid concentration in tissues. Albumin also binds and transports a variety of endogenous substances and medications. Serum albumin measurement is a reflection of hydration and nutritional status, blood oncotic pressure, liver and kidney function, and other chronic diseases, and the ratio of albumin in the CSF and serum can be used to evaluate blood brain barrier disorders. [Tietz] Urine albumin of less than 30 mg/day is considered normal. Microalbuminuria is defined as urine albumin of 30-300 mg/day and is an early sign of kidney damage from disorders such as diabetes or hypertension. Albuminuria is urine albumin >300 mg/day and is associated with overt kidney disease. PMID: 25512779
