Term Description

A Patient Plan of Care (PPOC) is a care plan that is individualized and mutually agreed upon with the patient/advocate. The care plan includes problem issues (diagnoses), expected healthcare outcomes, implementable interventions, and evaluation of progress toward outcomes based on follow up assessment. It is a framework to document critical thinking necessary for progressive evidenced-based outcomes.
