Myoglobin is a very sensitive marker (90-100%) for acute myocardial infarction (AMI) but lacks specificity. Serum concentrations of myoglobin increase above the reference interval as early as 1 hour after MI, with peak sensitivity in the range of 2-12 hr, suggesting that serum myoglobin reflects the early course of myocardial necrosis. Myoglobin is rapidly cleared and thus has a substantially reduced clinical sensitivity after 12 hr. For detecting AMI, myoglobin must be measured within the first 0-4 hr, the time period in which CK-MB and possibly cardiac troponin are still within their reference range.
