Cortisol/Cortisone [Mass Ratio] in Serum or Plasma
Active
Term Description
Apparent mineralocorticoid excess is a rare recessive genetic disease that is characterized by childhood onset of hypertension, hypokalemic alkalosis, and low plasma renin and aldosterone level. Hypertension is due to impaired conversion of cortisol to cortisone due to an inactive 11β-hydroxysteroid dehydrogenase. This condition may be diagnosed with elevated cortisol metabolite to cortisone metabolite ratios in urine or serum. It is recommended that cortisol/cortisone ratios (F/E) be tested in hypokalemic hypertension cases where aldosterone, deoxycorticosterone, and renin are all low. Elevated cortisol/cortisone ratios may be found in chronic renal failure, or excessive ingestion of glycyrrhetinic acid or derivatives, or in liver or bile-acid abnormalities.
Part Description
LP14161-1 Cortisol
Cortisol is a glucocorticoid produced in the adrenal cortex. A synthetic version of this same chemical is called hydrocortisone and manufactured as medication. Cortisol is not to be confused with cortisone, a closely related compound with much less (PubChem calls it "inactive") biologic activity. Cortisone is also offered as a drug, and it is converted to cortisol (the active form) in the liver. They both have a circadian rhythm with peak in the AM (around 8AM) and trough in the evening (around midnight), but this shifts according to sleep habits (work shifts). The morning measurement, which should be high, is most useful for diagnosing Addison's disease (too little cortisol) and the evening measurement, which should be low, is the most useful for diagnosing Cushing's disease (too much cortisol). Salivary cortisol may be a more sensitive measure than serum or plasma.
