A monocyte is a mononuclear leukocyte, intimately involved in the initiaton of an immune response. It also protects against blood-borne pathogens and migrates quickly to sites of infection in the tissues. Monocytes are usually identified in stained smears by their large bilobed nucleus. They are produced by the bone marrow from hematopoietic stem cell precursors called monoblasts. The monoblast is the first stage of monocyte maturation. The developmental stages (4 subtypes) of the monocytes are: monoblast, promonocyte, immature monocyte, and mature monocyte.

There seems to be some ambiguity regarding precise morphological definition of "immature monocyte" in the literature, but there have been attempts to clarify (Haematologica 2009;94:994-997.). There are considered 4 developmental stages beginning with monoblast, followed by promonocyte, then immature monocyte, then mature monocyte. From what can be gleaned from the literature, an immature monocyte would be called by having some less mature features(nucleus not as lobular, chromatin perhaps not as condensed, slightly more basophilic than mature, and somewhat smaller than mature). Evaluation of this lineage in blood and bone marrow can be indicative of leukemia type.
