Routine testing of Factor Xa inhibitor levels is not recommended because they have such a wide therapeutic index. Testing should only be done in special circumstances. Rivaroxaban LC/MS is measured using a validated liquid chromatography/mass spectrometry (LC/MS-MS) method. Use of liquid LC/MS-MS provides highly accurate measurement of rivaroxaban concentrations without the variable interferences associated with traditional clot-based and chromogenic assays. Studies performed using the LC/MS-MS method indicate that the assay is not sensitive to the presence of lupus anticoagulants or heparin administration nor to Factor VIII deficiency or the multiple factor deficiency associated with coumadin treatment.

LP135650-2   Rivaroxaban
Rivaroxaban is an oral anticoagulant, marketed as Xarelto. It is the first available orally active direct factor Xa inhibitor. Rivaroxaban is well absorbed from the gut and maximum inhibition of factor Xa occurs four hours after a dose. The effects lasts 8-12 hours, but factor Xa activity does not return to normal within 24 hours so once-daily dosing is possible.
