Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.

Mapping Guidance | LOINC | This term was made to accommodate a laboratory system outside of the US. LOINC currently discourages the use of specimens combining ser/plas and blood in one term (as ser/plas/bld) for two reasons. First, practices usually want to distinguish the two on ordering since whole blood usually implies a point of care test and ser/plas usually implies sending to another location. Second, whole blood tests are usually classified as low complexity (waived) under CLIA regulations, and because waived tests are excluded from the annual COLA report of testing volume (required by the Centers for Medicare and Medicaid Services) and ser/plas tests are not, the two have to be distinguished. |
