Measurement of baseline and stimulated C-peptide (after glucagon or a mixed meal challenge) provides a sensitive and clinically validated assessment of beta-cell function for type 1 diabetes patients. This is important as retention of beta-cell function in patients with type 1 diabetes is known to result in improved glycemic control and reduced hypoglycemia, retinopathy, and nephropathy. In the beta cells of the pancreas, the proinsulin molecule is cleaved to form insulin and C-peptide. C-peptide, a polypeptide consisting of 31 amino acids (MW~3000), is stored in the secretory granules of the beta cells and released into circulation in equimolar amounts with insulin.
