Term Description

Because Toscana virus is a known etiologic agent of meningitis, meningoencephalitis, and encephalitis, cerebrospinal fluid (CSF) may be tested. Specific antibodies for IgM are present during the acute phase of the Toscana Virus infection, while the presence of specific IgG antibodies in the absence of IgM indicates a previous infection. Cross reactions cannot be excluded with other correlated serotypes of Phlebovirus such as Sicily and Naples. This term was created for, but not limited in use to, the submitter's Diesse ENZYWELL TOSCANA VIRUS IgG/IgM test kit.

Toscana virus (TOSV) was identified in 1971 in central Italy from sandflies Phlebotomus perniciosus and Phlebotomus perfiliewi PMID: 24175218. TOSV belongs to the family Phenuiviridae, genus Phlebovirus [NCBI Taxonomy: 11590)]. TOSV is a serotype of the species Sandfly fever Naples virus PMID: 24175218. TOSV is the only neurotropic sandfly phlebovirus typically causing meningitis and encephalitis. Additional neuroinvasive manifestations such as Guillain-Barré syndrome, hydrocephalus, myositis, fasciitis, polymyeloradiculopathy, deafness, and facial paralysis, have been reported. The Toscana virus (TOSV) belongs to the Bunyaviridae family whose genome is made of three negative single-stranded RNA segments. There are two genotypes of TOSV: A, found mainly in Italy, and B, occurring mainly in Spain. The distribution of the two genotypes may indicate the involvement of different vectors. Infection by TOSV is most prevalent in Italy and Spain in the summer months, though the geographical region where TOSV is expanding includes other countries in the Mediterranean basin. TOSV is transmitted to humans by the Phlebotomus species of sandflies. Though the animal reservoir has not been identified, TOSV has been isolated from the brain of a bat, which raises the question of it possibly being a host to the virus. TOSV causes aseptic meningitis in 95% of the people it infects, meningoencephalitis in 4.5% of those infected, and an influenza-like illness in the remaining .5% who are infected. Recently, unusual symptoms associated with TOSV have included ischemic complications and hydrocephalus. TOSV is emerging as an important pathogen as its distribution increases due to changes in the climate, globalization and habitat modification. The Toscana virus (TOSV) belongs to the Phlebovirus genus of the Bunyaviridae family, and is an arthropod-borne virus transmitted to humans by Phlebotomus spp sandflies. The Toscana virus infection is characterized by fever, strong headache, and meningitis, or meningoencephalitis. TOSV is the major cause of aseptic meningitis (95%) and meningoencephalitis (4.5%) and influenza-like illness during the summer season. Some unusual clinical manifestations due to TOSV with severe sequelae, such as ischemic complications and hydrocephalus, have been reported. TOSV represents an important emerging pathogen and it is being investigated in several countries on the Mediterranean basin, including Italy, France, Spain, Portugal and Cyprus. Phylogenetic analysis has distinguished two genotypes of TOSV, A and B. The first is circulating mainly in Italy and the second in Spain, indicating a different geographic distribution possibly related to the vector.
