Dimethylarginines are endogenous analogues of L-arginine. N,N-dimethylarginine (also known as asymmetrical dimethylarginine or ADMA) and N,N'-dimethylarginine (symmetrical dimethylarginine or SDMA) both inhibit cellular L-arginine uptake, and ADMA also competitively inhibits nitric oxide (NO) synthases. ADMA is predominantly degraded by the enzyme dimethylarginine dimethylaminohydrolase, while SDMA is primarily excreted by the kidney. PMID: 25245031 ADMA and NO play a pivotal role in endothelial dysfunction, and elevated ADMA levels may be associated with a higher risk of cardiovascular disease. SDMA has not been associated with cardiovascular disease but correlates with both measured and estimated glomerular filtration rate and is a marker of renal function. PMID: 26021436

Creatinine or creatine anhydride, is a breakdown product of creatine phosphate in muscle. The loss of water molecule from creatine results in the formation of creatinine. It is transferred to the kidneys by blood plasma, whereupon it is eliminated by glomular filtration and partial tubular excretion. Creatinine is usually produced at a fairly constant rate and measuring its serum level is a simple test. A rise in blood creatinine levels is observed only with marked damage to functioning nephrons; therefore this test is not suitable for detecting early kidney disease. Creatine and creatinine are metabolized in the kidneys, muscle, liver and pancreas.
