Changes in the CSF, plasma and urine levels of beta alanine, beta aminoisobutyrate (BAIB) and gamma aminobutyrate (GABA) may help identify certain metabolic disorders. Beta alanine is a structural analogue of GABA and may also function as a neurotransmitter since it is both a glycine and GABAA receptor agonist. Dihydropyrimidine dehydrogenase (DPD) is the first enzyme in the pyrimidine degradation pathway, in which uracil and thymine are catabolized to beta alanine and R-BAIB, respectively. The S enantiomer of BAIB (S-BAIB) is derived from valine catabolism. Under normal circumstances, urine contains predominantly R-BAIB (from thymine) because it is eliminated by filtration and tubular secretion, while plasma contains about 80% S-BAIB (from valine), which is reabsorbed in the kidneys. In patients with a DPD deficiency, uracil and thymine catabolism is decreased. This results in mildly decreased plasma and urine concentrations and normal CSF concentration of beta alanine, and moderately decreased CSF concentration and markedly decreased plasma concentration of BAIB. On the other hand, urine BAIB is elevated, which indicates that there must be cross-over in the thymine and valine degradation pathways. PMID: 14705962
