Term Description

The episode of care primary diagnosis code is the billing code that is listed first on the claim form for the most recent visit in a given episode of care. This term was created for, but is not limited in use to, the the American Physical Therapy Association's Physical Therapy Outcomes Registry, for which the diagnosis code will almost always be reported using ICD-10-CM.
