Tetrahydrocorticosterone/Allo-tetrahydrocorticosterone [Molar ratio] in Urine

Term Description
The quantitative ratios of various steroids in urine are measured to diagnose conditions such as 17-alpha-, 21-, and 11-beta hydroxylase deficiencies, pseudohypoaldosteronismus, 3-beta hydroxysteroid dehydrogenase deficiency, corticosterone methyl oxidase deficiency, 11-beta hydroxysteroid dehydrogenase types 1 and 2 deficiency, and 5-alpha reductase deficiency. The results along with clinical symptoms and past medical history are used for the assessment and treatment of the associated deficiency or condition. Below are examples of various disorders and their associated urinary findings:
1. 17-alpha hydroxylase deficiency: (THA+THB+5aTHB)/(THE+THF+5aTHF) > 10, (THA+THB+5aTHB)/(AN+ET) > 10.
2. 21-hydroxylase deficiency: 17HP/(THE+THF+5aTHF) > 0.25, PT/(THE+THF+5aTHF) > 0.25.
3. 11-beta HSD2 deficiency: (THF+5aTHF)/THE > 3
4. 11-beta HSD1 deficiency: (THF+5aTHF)/THE < 0.1
5. 11-beta hydroxylase deficiency: 100*THS/(THE+THF+5aTHF) > 10, 100*THDOC/(THE+THF+5aTHF) > 10
6. Pseudohypoaldosteronismus: 100*THALDO/(THE+THF+5aTHF) > 5
7. 5-alpha reductase: ET/AN > 5, THF/5aTHF > 10
Where AN = Androsterone, ET= etiocholanolone, 17HP= metabolites of 17-OH-progesterone (pregnandiol, pregnantriol, 17-OH-pregnanolone), THA = tetrahydro compound A, THB = tetrahydrocorticosterone, 5aTHB =5 alpha tetrahydrocorticosterone, THE = tetrahydrocortisone, THF = Tetrahydrocortisone, 5aTHF = 5a-tetrahydrocortisone or Allo-tetrahydrocortisone, THS = tetrahydrodeoxycortisol, THALDO = tetrahydroaldosterone, and THDOC =tetrahydrodeoxycortisone

Beta-Cortol is a normal androgen metabolite present in adults. It has been found in the urine of infants as well. Beta-Cortol is the 5b enantiomer of beta-allocortol. Beta-cortol levels are significantly higher in premenopausal women with leiomyomas than in age-matched healthy premenopausal control women. Uterine leiomyomas are tumors closely associated with estrogen levels and it has been noted that the development of leiomyomas depends on the condition of menstruation, perimenopause and pregnancy.
