The measles virus (MeV) is a single-stranded, enveloped RNA virus and a member of the Paramyxoviridae family. Measles was first discovered to be caused by an infectious agent in 1757 by a Scottish physician. After becoming a notifiable disease in the United States in 1912, there were roughly 6,000 deaths reported each year in the following ten years due to MeV. The vaccination against MeV become available in the United States in 1963 which caused a rapid drop in MeV infections. MeV is highly contagious and is transmitted through infected secretions. Symptoms of infection include fever, malaise, cough, coryza, and conjunctivitis. Later, pathognomonic enanthema (Koplik spots) develop and are followed by a maculopapular rash that appears 14 days after exposure to MeV and spreads from the head, down the trunk, and to the lower extremities. Patients susceptible to serious complications may experience acute encephalitis and subsequent brain damage, fatal respiratory and neurologic complications, or a rare, fatal degenerative disease of the central nervous system characterized by behavioral and intellectual deterioration and seizures that begin 7-10 years after the measles infection. There are 24 genotypes of MeV recognized by the World Health Organization that are categorized by the nucleotide sequences of their hemagglutinin (H) and nucleoprotein (N) genes. The region of the N gene that distinguishes between the genotypes is a 450 nucleotide sequence of the carboxyl terminus referred to as N-450. Sequencing the N-450 region can identify the genotype of the infecting virus. Because infection by MeV vaccine is sometimes given as a post-exposure prophylactic, it has become important to differentiate MeV wild-type infections from the virus present in the vaccine.Vaccine strains all belong to genotype A and the wild-type genotype A MeV strains are believed to no longer be circulating. That being the case, if a patient tests positive for genotype A, then the virus is most likely due to the vaccine.Also, the region of the N gene between nucleotides 478 and 548 is identical for all genotype A vaccine strains that have been analyzed, and PCR-based assays have been developed based on that region so that genotype A vaccine strains can be identified without having to sequence the entire gene.[https://www.cdc.gov/measles/hcp/index.html][http://jcm.asm.org/content/55/3/686.full]
