Von Willebrand disease (VWD) is caused by either a qualitative or quantitative deficit of the multimeric glycoprotein known as von Willebrand factor (VWF). VWD affects 1-2% of the world population, making it the most common inherited bleeding disorder. VWF plays a crucial role in primary hemostasis by triggering platelet adhesion to injured vessel walls and promoting platelet aggregation. VWF also protects factor VIII from premature degradation in the peripheral circulation. To diagnose VWD, there are several tests that must be run including Ristocetin cofactor activity, VWF antigen quantitative assay, Factor VIII activity, and collagen binding assay. Additional tests are needed to determine the subtype of VWD. The collagen binding assay measures the ability of VWF to bind to collagen by combining patient plasma with collagen that is adhered to a matrix. Then anti-VWF and dye are added in order to measure how much patient VWF combined to the collagen. When an antibody binds with an epitope on the VFW that is meant to bind to collagen, the ability of VWF to function is compromised. The collagen binding assay evaluates the functionality of the VWF, and is reduced in the presence of inhibiting anti-VWF antibodies. Patients who are most likely to develop anti-VWF include patients with complete or partial VWF gene deletions, patients who have received multiple transfusions, and patients with a family history of developing anti-VWF antibodies.
