Legionella pneumophila, a gram-negative bacillus, most commonly causes acute pneumonia (legionellosis). Transmission to humans is usually through aerosols of infected water sources such as fountains, misting systems, and drinking water. Severity of infections due to L. pneumophila range from mild (walking pneumonia) to fatal (multilobar pneumonia). Antimicrobial therapy usually includes erythromycin, with the addition of rifampin in severe cases. Initial response to therapy is usually rapid, but full recovery can take weeks to months.
