Term Description

This term represents a collection of information that would be reported to public health authorities about a patient who has or potentially has been exposed to SARS Coronavirus 2 (SARS-CoV-2) and may have or is diagnosed with having Coronavirus disease 2019 (COVID-19). This term does not represent a clinical COVID-19 case report that might be published in the medical literature or elsewhere. The term was created for, but not limited in use to, the CDC Human Infection with 2019 Novel Coronavirus Case Report Form [https://www.cdc.gov/coronavirus/2019-ncov/downloads/pui-form.pdf], and can be used for other COVID-19 case report forms, such as the WHO Revised case report form for Confirmed Novel Coronavirus COVID-19 [https://www.who.int/docs/default-source/coronaviruse/2019-covid-crf-v6.pdf?sfvrsn=c5ff90c6_2].
