Description
Measures circulating epithelial cells that are assumed to be carcinoma cells. In the context of breast cancer, a count of greater than 5 circulating tumor calls per 7.5 mL predicts a significantly shorter survival than those with a less then 5. Test is based on immunologic stain for Epithelial Cell Adhesion Molecule (EpCAM) antigen which are manetically tagged to concentrate them, then stained with CD45+ Alophycocyan (CD45-APC), anti cytokeratin phycoerythrin 8, 18, and 19 (CK-PE) and PAPI to identify the nucleus. The goal is to find epithelium derived cells. EpCAM is not always present in tumor cells and it is not completely specific so CK-PE is needed to confirm epitelial, PAPI to verify that the object is a cell not depre and CD45 to detect and exclude white cells. In terms of markers, the cells of interest are those that EpCAM+ CK-PE+ PAPI+ (to be sure that they are cells) and CD45-APC negative (to exclude white cells. Counts from this manufacturer are reportes per 7.5ml.
At present only one product is approved by the FDA is ."CellSearch™ system (Veridex), and it is offered by all of the major laboratories. As of May 2011, It has only been approved for metastatic , breast, prostate and colorectal cancer. The test has not specificity for a particular cancer, but the cut off for deciding worse prognosis differs by organ. Other mechanisms for measuring Circulating Tumor cells , exist, most internally developed by laboratories and not fully automated. Another company has an experimental system that does not depend upon magnetically tagged EpCAM for a first pass cut- claims to be able to count more cells (30 cc of blood), and may be more sensitive.
