The C1 esterase inhibitor assay is used to diagnose hereditary angioneurotic edema. Exhaustion of C1 esterase inhibitor allows spontaneous auto-catalytic C1 activation and the break down of C2 and C components. The disease can be caused by low concentrations or the abnormal structure of the protein. The disease is inherited as an autosomal dominant trait and has been successfully treated with prednisolone and transfusion of fresh frozen plasma. Source: Regenstrief Institute
In congenital form of angioedema there is a deficiency in C-1 esterase inhibitor detectable by nephelometry. Acquired forms are usually associated with qualitative (functional) deficiency of the inhibitor. This is best detected by the enzyme immunoassay for the functional component. To detect acquired forms of angioedema due to deficency of C-1 esterase inhibitor (C1-INH), a multispecific, protease inhibitor that is present in normal human plasma is measured.
