Description

Normally bilirubin is thought to enter CSF only by passing through the blood brain barrier bound to albumin. This allows us to predict an "expected" CSF bilirubin concentration. Since bilirubin is a breakdown product of oxyhemoglobin, the bilirubin excess in CSF can be used to predict subarachnoid hemorrhage in the absence of cranial CT findings. Billirubin excess is defined as the surplus CSF bilirubin concentration measured after subtraction of an estimated upper limit for the individual patient. This upper limit, called the ''CSF bilirubin limit'' (CBL), is a calculated by multiplying the serum bilirubin concentration times the albumin quotient: ([bilirubin]serum X [albumin]CSF/[albumin]serum) X 1.25."
