Description

Forms of high molecular weight insulin-like growth factor-II ('Big IGF-2' and 'Big-big IGF-2') represent partially processed pro-IGF-2 and have a molecular weight of 10-18 kD in contrast to mature IGF-2, which has a molecular weight of 7.5 kD. The 10 and 12 kD high molecular weight forms are known as 'Big-IGF-2' and the 15 and 18 kDa forms are known as 'big big-IGF-2'. Increased amounts of high molecular weight IGF-2 forms is present in patients with various forms of tumor-related hypoglycemia, including extrapancreatic tumor hypoglycemia (EPTH) and non-islet cell tumor hypoglycemia (NICTH). NICTH is characteristic of Doege-Potter syndrome (DPS), a paraneoplastic syndrome in which hypoglycemia is associated with the presence of one or more non-islet tumors in the pleural cavity.
