Description

Cyclosporine (Brand names Neoral, Restasis, Sandimmune, Gengraf) is a cyclic undecapeptide of fungal origin and a potent calcineurin inhibitor immunosuppressive agent. The introduction of cyclosporine in human organ transplantation substantially improved patient and graft survival in patients receiving heart, kidney, liver, pancreas, lung or bone marrow transplants. Cyclosporine was the first drug to specifically and reversibly inhibit the activation and proliferation of lymphocytes. The main mechanism through which cyclosporine exerts its immunosuppressive effect is the inhibition of T cell activation and proliferation. Monitoring and concentration-controlled dosing is required to maintain patient drug levels within a narrow therapeutic window; low levels could cause transplant rejection and high levels may cause nephrotoxicity.
