Plasminogen Activator Inhibitor 1 (PAI-1) is an acute-phase reactant that inhibits plasminogen activators. Tissue plasminogen activator (tPA) converts inactive plasminogen into plasmin during fibrinolysis. Plasmin degrades fibrin and is critical to fibrinolysis. PAI-1 is the primary inhibitor of tPA and limits the production of plasmin, thus controlling fibrinolysis. Uncontrolled plasmin production can result in increased bleeding. Increased PAI-1 levels are seen with infection, inflammation, and trauma. Increased levels are associated with increased risk of acute coronary syndrome. Patients with chronic and acute coronary artery disease and patients with restenosis following coronary angioplasty may also have increased levels of PAI-1.
